A complete record of your care and treatment is kept in our medical records department. These records are strictly confidential, and only authorized persons have access to them.
Copies of your health records will be provided upon written request and for a reasonable fee. To request copies of your medical records, please write a letter and mail it with a copy of your driver’s license, to the address below. Please include your telephone number so we can contact you.
